Privacy Policy and Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can access this information.

Mama’s Acupuncture, PLLC is required by law to maintain the privacy and confidentiality of your protected health information (PHI) and to provide you with notice of its legal duties and privacy practices with respect to that information.

In the course of administering your care, we may collect and maintain information that includes non-public personal and medical data, such as billing transactions, medical history, treatment notes, test results, and any written or verbal communications with other healthcare providers, insurance companies, Worker’s Compensation, your employer (in applicable cases), and other third-party administrators (e.g. medical record requests, claim processing information).

We may also need to share limited personal, medical, and financial information with your insurance provider, Worker’s Compensation, or with other medical professionals upon your authorization.

Please note: Appointment scheduling and administrative functions are securely managed through Jane, a third-party, HIPAA-compliant platform. By booking through Jane, you acknowledge and consent to the transmission of necessary personal and health-related information for the purposes of scheduling, appointment reminders, and basic intake.
